Required permission and waiver forms

FOR YOUNG MEN
The Permission and Medical Release Form, Trinity Forest Adventure Park Waiver, and Trinity River Audubon waiver are required to be completed before the activity. 

*The Permission and Medical Release Form for the Church AND the Trinity River Nature Preserve Waivers must be printed and submitted to a member of the Stake YM Presidency. This can be done in advance, or brought to the activity with you.
